The OC Form is what everyone needs to fill out to be considered for the the RP. You might be wondering, what does a filled in sheet look like? What does a good sheet look like? What do I need to do differently for my alien character, or my human character? To remedy some of these questions, we've left two examples of a filled in sheet below.

These two examples represent a "Pure Human" OC, (A human character with no supernatural abilities who can wield three Nanos,) and an OC with non-human Origins, (an alien, demon, magic-user, etc.)

We're going to start off with the "Pure Human" OC form, for my character Jasper Leeblossom. I will include all of the optional sections as well.

EXAMPLE 1
Name/Identification/Alias: Jasper Leeblossom
[This one is easy, all you need to do is type your character's name. You can use a nickname, codename, or whatever it is your character is referred to by. Jasper's first and last name work here.]

Age: 21
[This one is fairly easy as well. Simply put the age of your character here. If your character is a different age where they come from compared to Earth years, you can include that.]

Appearance: Jasper has messy brown hair that he keeps cut short. He has a full, but trimmed, beard that matches the same brown of his hair. Jasper has tanned white skin, and hazel eyes. He is 6'5" tall, and has a surprisingly muscular physique, showing that he frequents the gym. He typically wears a button-up, red flannel shirt, with the sleeves rolled up to his elbows, pairing them with dark and tight jeans and matching shoes. During battle, he wears a black and red colored armored suit.
[Your character's appearance description doesn't need to be anything too fancy. Here, I only really covered the basics of his hair color and general outfit, tucking a few more details in. You'll want to make sure you can paint a picture of what your character looks like with your description, and if you can't, using an image works too!]

Race/Species: Human
[Another self-explanatory section, this is where you'd put the race of your character. Jasper is a human, so human is what you'd leave here.]

Alignment: Neutral Good
[I, personally, like to use the DnD Morality chart to dictate my characters' alignment, but a simple "good," "hero," "bad," "neutral" works just as well here.]

Affiliation: -OPTIONAL- Foster's Home for Imaginary Friends
[The first optional section on the form, this is where you'd put who/where your character works under, or who their loyalties lay. This isn't restricted to the pre-established organizations, and you can include whatever you think is right for your character. Jasper spends a lot of time at Foster's protecting the friends and is close with several characters from there, so saying he's affiliated with the home makes sense.]

Origins: Human
[Since Jasper is a normal human with no magical/supernatural abilities, he technically has no origin. All pure human characters would just leave human here.]

Powers: N/A
[Character with Human Origins should always leave this section with N/A. Pure humans have no powers.]

Nanos: -OPTIONAL- Jasper prefers the Nano Powers of Drain, Run, and Rocket. When given the option, he likes using Nanos of Professor Utonium, Numbuh Four, and Hex.
[Another optional section. Jasper is a pure human, which means he can carry up to three Nanos with him at a time. You don't need to list which Nanos your character use, but you can if you'd like. Remember, these are just the powers you feel your character would use the most, not the only powers they can use.]

Strengths: Jasper was a deliquescent during his teenage years, and has gathered a rather extensive knowledge of Townsville's inner streets. This makes traveling around that city easier for him. Jasper is fueled by a passion to protect his family, caring the most for his mother and sister and willing to drop everything to save them. Jasper's physical strength can also prove to be a strength for him, giving him an intimidating look and giving him the upper hand in any physical altercations he can get in.
[It's not as easy as just listing out three positives to make strengths. You'll want to provide at least a sentence of explanation for each strength, and tell us why this quality is helpful to them. Jasper's negative past is turned into a positive with his first strength, something you can think about having your character relate to.]

Weaknesses: This big, burly man has a soft spot for the tiniest, cutest of animals and Imaginary Friends, typically entering a trace while being captivated by their cuteness. Jasper's sister often calls him immature for this trait, and he's left defenseless in this state. Jasper is extremely hot-headed, and doesn't think about his actions in the heat of the moment. His impulsive actions can lead to a world of hurt for either party, typically himself. His strength of wanting to protect his family is knocked down a peg as his feelings grow more intense. Jasper is arguably more reckless with his decisions if he believes his family is in any immediate danger.
[Just as with the strengths, you'll want to explain each weakness a bit and tell us why they hinder your character. I, personally, love to tie one of my strengths to a weakness to balance out a character. Jasper's love for his family makes him strong, but that love can also lead to impulsive and unsafe decisions.]

Likes: -OPTIONAL- Jasper is a fan of working out, obvious by his looks. Before the invasion, he frequented the gym, and now trains his body by fighting Fusions. Jasper's favorite pastime is volunteering at Foster's Home and spending time with all of the Imaginary Friends.
[Likes and Dislikes serve more as sections to help you than they are to help us. These simple choices can really help you flesh out a character and make them unique.]

Dislikes: -OPTIONAL- Jasper is not a fan of teenage boys, namely the ones who want to talk to his little sister. His reasoning is that he was once a rude and disrespectful teenager, so he knows what they're capable of. Going in hand with this, Jasper also doesn't like it when his sister doesn't listen to him and his warnings.
[There's no limit on how many/less Likes and Dislike you want to leave. Whatever makes you feel confident is good enough. If you are filling out these sections, try to make them different from the Strengths/Weaknesses.]

Personality: Jasper Leeblossom was once a crude and snarky hooligan that ran the streets of Townsville. As an adult, Jasper's cleaned up his act and turned his personality on its head. Unlike as a teenager, Jasper likes to step up and take responsibility for his actions, and can be seen smiling while teaching these responsibilities to others. His charm is unmatched, being able to talk just about anyone into listening to his self-righteousness.

His happy-go-lucky outlook on the rest of his life is soured by his aggressive and impulsive attitude. While not as bad as when he was younger, Jasper can be extremely abrasive, and will most likely take every pointed comment as an attack on his character. Unfortunately, Jasper loves to fight when he gets into these moods, and will often encourage the other party to attack him, or start swinging himself.
[A good personality includes pros and cons. Remember what Hannah Montana said, kids; Nobody's Perfect. Your character should be a well balanced person with flaws. You don't need to leave a novel, a single paragraph works fine, but being more detailed might help you.]

Biography: Well before the invasion, Jasper was born to his mother Diana and late father, Dylan, who passed shortly after his birth. When his mother remarried, Jasper was adopted by his step-father and took the last name Leeblossom. He was a respectful child for the most part, and was always a good brother to his little sister once she was born.

When Jasper reached his teenage years, typical puberty hormones made him start acting out, growing into the abrasive personality he holds today. In efforts to fit in with the ones he looked up to, he started to run with some shadier figures. For the remainder of his teenage years, Jasper was known to be a hoodlum with a bad attitude. Jasper's mother, Diana, loved her son no matter what he acted like, but his sudden shift in personality didn't reflect on her parenting well.

After turning 18, Jasper developed a crush on a girl he went to school with, Frankie Foster, and started volunteering at her grandmother's foster home for Imaginary Friends in order to spend more time with her. These would be the days where he starts to leave the hood-rat ways behind him, growing softer at the sight of the friends he helps.

The next few years saw Jasper return to Foster's regularly, not just to see Frankie, but to help out even if she wasn't there. It became obvious spending time there was something he actively enjoys doing, even bringing his sister along with him sometimes.

He was at Foster's during the invasion, but ventured out into the warzone to find his sister, who was safe. Since then, he's been combining his previous street smarts with his newfound caring skills to protect those he loves from the Fusions, as well as the Imaginary Friends. He currently lives with Frankie downtown.
[A backstory doesn't need to be a novel-length long, but it should be a decent enough size to show that you've really thought out the character and thought about the details. Jasper's history here isn't anything too grand, and is about as simple as we want. A description of their early life, what they like to do in life, and how Planet Fusion has impacted them.]

Inventory: -OPTIONAL- Jasper always has his car-keys and NanoCom on him. He is always equipped with a simple pistol for protection against the Fusions.
[The final optional section, and the final section, your inventory is mostly for you as the roleplayer. Of course, you don't need to list all the things your character has, just the essentials they'd use regularly. I listed Jasper's car keys to show the love he has for his car. If your character doesn't have powers, it would be a good idea to list which weapons they carry.]

The next example is that of a character that isn't a pure human, such as a Magic user, Demon, etc. We're using an alien in this example. I will not be including the optional sections in this example.

EXAMPLE 2
Name/Identification/Alias: Roxanne "Roxie" Leeblossom
[Roxie's full name is Roxanne, but she only goes by Roxie. Typically, if your character only goes by a nickname, you wouldn't have to leave a full name here.]

Age: 16
[Not much to say here I didn't already say. This is how old your character is.]

Appearance: Roxie, who stands at 5'11" tall, has long blond hair that reaches her mid back. She mostly keeps it tied up in a ponytail. She likes to sport a pink t-shirt that splits to reveal a black undershirt. She also wears short shorts, which her mother doesn't approve of but Roxie still bought them anyways. If it's colder, she can also wear a purple coat too. All of her shirts and clothes have four holes in the back so Roxie can poke her four large spiderlegs out. These spiderlegs are purple in color and have sharp points, which are great for poking her enemies. They protrude from her back, and are her real limbs.
Image:
[Again, this doesn't need to be too fancy. Roxie has alien appendages, so I made sure to detail them. I also included a photo of Roxie's appearance, drawn by the artist from the old FusionFall Universe projects, Virt. You don't have to include a description AND a photo, just one or the other.]

Race/Species: Half-Human/Half-Arachnichimp Hybrid
[Roxie is only half-human. Characters that are hybrids of two species should list both species here. If your character is an alien, you can leave the name of their species, or just simply "Alien."]

Alignment: Chaotic Good
[Just like with Jasper, I've used the DnD Morality chart to describe Roxie's alignment. Roxie is a good guy with a bit of an attitude that puts her in the chaotic category.]

Origins: Alien (Arachnichimp)
[Roxie's powers come from her alien half, so she has Alien Origins. You are not required to put the name of your alien species here.]

Powers: Roxie's spiderlegs give her the ability to walk on walls and ceilings, (Medium). This is relative to her strength and stamina. She has the ability to create sticky and gooey spiderwebs, which she can shoot from the tips of her spiderlegs, (High). She is able to use these webs to swing around town, much like the Marvel superhero, Spider-Man. While not exactly a power, Roxie's spiderlegs are extremely sharp, and can easily stab through skin.
[Every power listed must have a power level included, Low, Medium, or High. Roxie's ability to walk on walls is a Medium power, since it relates to her current strength and stamina. If she can't stand up normally, she wouldn't be able to stand on a wall. Her sharp spiderlegs aren't really a power, which is why there is no power level.]

Strengths: Roxie's physical abilities put her ahead of others, giving her an upper hand in most situations. She knows how to use her alien limbs and spiderwebs, and knows how to use them well, as she can quickly swing herself into a position with an advantage in the blink of an eye. Roxie's preferred weapon is actually a Shattergun, rather than her alien abilities. She's surprisingly skilled with the weapon, for someone of her age, of course, and is great at explaining her favorite techniques. When she needs to, Roxie can channel her inner spider and become extremely sneaky. This is helpful for maneuvering dangerous environments, but also for escaping a chase.
[Again, you'll want to tell us why a strength is just that, a strength for your character. Roxie's alien DNA gives her an advantage, but that goes without saying. That's why I explained what exactly about her alien side gives her the upper hand.]

Weaknesses: Roxie had an extremely traumatic experience during the initial invasion, leading to her now being nearly unable to witness someone getting hurt. She'll start to lock up, and inadvertently put herself in harm's way if she sees someone being hurt, let alone if the victim is someone she knows. In addition to this, she struggles with being alone and feeling alone. The trauma from the invasion makes her feel like she has a target painted on her back if she's alone, and often times she is attached to somebody's hip. On the opposite end of the spectrum, Roxie has a very abrasive side to her personality, which doesn't always bode well for her in social situations. Often times, Roxie is seen as the "mean girl."
[Roxie lost most of her friends during the invasion. A traumatic experience like this can lead to a multitude of weaknesses that can affect a person. A weakness shouldn't always completely debilitate a character, like Roxie's first weakness, but it should be enough to counter your character's abilities.]

Personality: Roxie can either come off as the most lovable person you could meet, or the biggest witch on this side of Townsville. Before Fuse invaded and she was in school, Roxie was the talk of the halls, being the Queen Bee of her class. Unfortunately, not all of her classmates loved her because of her personality. A few were scared of her, but can you really blame her classmates? Roxie wasn't stupid, and knew that people were scared of her spiderlegs. Sometimes, she can use this to her advantage to get what she wants.

Roxie actually has a soft heart and could be a model citizen if she didn't have an abrasive side. Her soft side is shown off when Roxie helps her neighbor Professor Utonium with his many scientific works and when she visits Foster's Home for Imaginary Friends and spend time with all the friends. Roxie also made it a habit to visit her grandparents in Nowhere often, if not just to see her favorite dog, Courage.

Roxie, especially after Fuse's initial invasion, has a bit of a seniority complex, usually self-appointing herself as the leader of the current group she's in. Because of this coupled with Roxie's general bossy attitude and general stubbornness, it's not hard to see why some people are not the biggest fans of her. Despite this, Roxie overloads those closest to her with love, care, and affection as she thinks the most important thing is letting those she loves know that she loves them too and she cares and supports them.
[So I've had Roxie around for awhile, so these next two slots may be packed. Giving examples on how your character expresses their personality is a great idea, and can really paint a picture of how your character acts.]

Biography: Roxie looks like your normal, everyday teenage girl... Except for the fact that she has four, large spiderlegs sticking out of her back. Roxie's father, Chester Leeblossom, was part-Arachnichimp, and her mother was a simple human, Diana. Chester mysteriously disappeared short after Roxie was born, leaving Diana to raise her alongside Jasper.

Roxie lived a fairly typical life, growing up in Pokey Oaks and being raised as a human. Her spiderlegs weren't too much of a distraction, since the world has been accustomed to aliens and those with super powers for awhile. Despite this, she was the subject of minor bullying growing up. By the time she reached middle school, Roxie started to utilize her alien limbs for fear, turning away the bullies, but developing a bit of an bully trait herself.

Roxie was a freshman at the newly destroyed Pokey Oaks High School, and was there during the invasion. She lost a good number of her friends in the chaos, only rescued by the K.N.D. and her brother. Since then, Roxie has been struggling with depression, and her abrasive side has begun to grow. To her mother's dismay, both Roxie and her brother have volunteered to fight against Fuse. Roxie shocked her neighbors, the Utonium's, when she picked to follow Mojo Jojo as a Guide.
[Two paragraphs is probably a good size for a biography/history. It's not always important to detail the names of all your character's family members, but it's not a bad idea to include if you have fleshed your character out that much. You should be able to tell us why your character is choosing to fight back against Fuse in your history.]

And there we have it. These are just examples, and you can make your character into whatever you'd like, as long as it follows the rules.
